Sample ARM Disclosure

Below are links to a sample ARM disclosure form to modify for your credit union's use. If you already have an appropriate disclosure, by all means use it. Although a substantial amount of care went into constructing the example, you should of course secure legal advice on the form you actually use. If your margin will be other than 2.75%, or if your program will have other terms that need to be disclosed, you will of course need to modify the form.
